29 CFR 1910.22(c): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc.: A.) Mattocks Five, Inc. dba Albion Mill, 73 Canal Street, Albion, PA 16401, second floor area-bin access area, on or about February 10, 2016 and times prior thereto: Employees were exposed to fall hazards of approximately 7.7 feet while accessing various bins on the 2nd floor. Entry and exit access openings of the bins were not protected by a guardrail or similar in order to protect employees from falling into the bins.
