29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. a) Jobsite - Employees engaged in framing and welding operation at heights greater than six (6) were exposed to fall hazards through unprotected floor holes without the use of a fall protection system. Observed on or about 29 January 2016.
