29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Instance 1#. On or about 12/31/2015, Navy Operational Support Center (NOSC) Building Addition, Ft. Buchanan, PR - building interior area: Employees were using a ladder installed on to scaffold metal frame which did not extend 3 feet above the upper landing surface to gain to the scaffold working area. Instance 2#. On or about 1/27/2016, Navy Operational Support Center (NOSC) Building Addition, Ft. Buchanan, PR - building interior area: Employees were using a ladder installed on to scaffold metal frame which did not extend 3 feet above the upper landing surface to gain to the scaffold working area.
