29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): A) Menards, Inc., located at 14502 CR 15, Holiday City, OH 43554: On or about January 12, 2016, the employer did not remove the Toyota forklift # 7367 (Model 7FBEU20, Serial # 13646) from use based on mechanical defects such as, but not limited to, defective back-up light and one of the brake lights. Employees were exposed to struck-by and caught-in-between hazards. B) Menards, Inc., located at 14502 CR 15, Holiday City, OH 43554: On or about January 12, 2016, the employer did not remove the Toyota forklift # 8100 (Model 7FBEU15, Serial # 22017) from use based on mechanical defects such as, but not limited to, defective strobe light. Employees were exposed to struck-by and caught-in-between hazards. C) Menards, Inc., located at 14502 CR 15, Holiday City, OH 43554: On or about January 12, 2016, the employer did not remove the Toyota forklift # 8244 (Model 7FBEU15, Serial # 25293) from use based on mechanical defects such as, but not limited to, defective strobe light.
