29 CFR 1926.452(j)(2): Poles shall be secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. (a) Location - 143 Chestnut Street, Lynn, Massachusetts: On or about January 7, 2016, employees installing roof shingles from a pump jack scaffold, the pump jack poles were not secure at the bottom to prevent unintentional displacement.
