29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft; a) On or about 01/06/16, in the Coal Handling Building; two employees greased and lubricated the coal elevator #2 and were exposed to caught in hazards when they perform these activities while the coal elevator was operating and without either employee affixing a lockout device on the energy isolation device. b) On or about 01/05/16, in the Battery Department; two employees greased and lubricated the pusher and were exposed to caught in or caught between hazards when they performed these activities while the pusher was energized and without either employee affixing a lockout device on the energy isolation device.
