29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: a/ The employer is failing to protect employees from amputation hazards. This condition was last observed on and about 02/17/16, at 2 Industrial Drive, Piedmont, MO 63957. Lockout/tagout devices are not provided to operators who are required to service the ILA-PAK Bagging Machine, m/n OF2000, s/n 074111001. These devices would be required to isolate the electrical and pneumatic energy sources belonging to this machine.
