29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about December 3, 2015, and times prior thereto, employees in the fabrication area were exposed to a struck-by hazard when operating a Press Well notching machine, Model NM80, ID# 1, without one or more methods of machine guarding to protect the operator from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. b. On or about December 3, 2015, and times prior thereto, employees in the fabrication area were exposed to a struck-by hazard when operating a Press Well notching machine, Model NM80, ID# 2, without one or more methods of machine guarding to protect the operator from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. c. On or about December 3, 2015, and times prior thereto, employees in the fabrication area were exposed to a struck-by hazard when operating a Press Well notching machine, Model NM80, ID# 3, without one or more methods of machine guarding to protect the operator from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. d. On or about December 3, 2015, and times prior thereto, employees in the fabrication area were exposed to a struck-by hazard when operating a Press Well notching machine, ID# NOT3, without one or more methods of machine guarding to protect the operator from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. e. On or about December 3, 2015, and times prior thereto, employees in the fabrication area were exposed to a struck-by hazard when operating a Ficep Ironworker punch, Model TACO 1050 PLC/SP 100, S/N 33544-Punch 3/C, without one or more methods of machine guarding to protect the operator from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. f. On or about December 3, 2015, and times prior thereto, employees in the fabrication area were exposed to a struck-by hazard when operating a Ficep Ironworker punch, Model TACO 1050 PLC/SP 100, S/N 33546-Punch 2/B, without one or more methods of machine guarding to protect the operator from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. g. On or about December 3, 2015, and times prior thereto, employees in the fabrication area were exposed to a struck-by hazard when operating a Ficep Ironworker punch, Model TACO 1050 PLC/SP 100, S/N 33547-Punch 1/A, without one or more methods of machine guarding to protect the operator from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.
