29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: IN WASH YARD: On or about December 1, 2015, and at times prior thereto, employees were exposed to potential oxygen deficient atmospheres and/or inhalation of toxic gases or vapors, when required to wear tight-fitting 3M 6000 half-facepiece respirators with Organic Vapor/Acid Gas/P100 cartridges without the protections afforded by a respiratory protection program. Employees routinely entered trucks and tanks for cleaning operations that were contaminated with waste from oil and gas well hydraulic fracturing operations.
