OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: A.) Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA, 15824, production area, on or about November 23, 2015 and times prior thereto: Employees are exposed to amputation / struck-by hazards from a lack of powder metal press inspections. Various powered metal presses used for compacting powdered metal and sizing parts had deficiencies due to a lack of inspections. These deficiencies included, but were not limited to: 1.) Missing and inadequate guarding of the point of operation 2.) Guards that are easily removal 3.) Incomplete and/or inoperable safety devices 4.) Missing guards around the powder metal press flywheel 5.) Missing guarding to prevent exposure to the moving parts at the rear of the presses 6.) Malfunctioning braking systems 7.) Inspections of the press braking systems were not conducted
