29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Construction site, American House, Perkasie PA - The employer does not ensure that floor holes are not properly guarded. This continuing violation was most recently observed on November 2, 2015, when employees framing the three story structure worked and walked in close proximity to a hole in the floor of approximately 2 feet by 4 feet that was used as access to an upper working level. The employer's ongoing obligation is to ensure that any floor holes are protected to prevent workers from falling to a lower level.
