Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Crushing and possible death from the hazards involved with hoists and heavy material (engines) falling from material handling equipment in the salvage area in 45 Basement where the bolts on the end stops on the monorail were very loose and in a shear position which could allow the connection to break and the load to fall. Again in the machine repair area press shop a Caroling 5000 cherry picker had a lifting hook connected to the chain by means of a 1/4 inch screw and bolt and some washers. Among other methods, one feasible and acceptable abatement method to correct this hazard is to inspect and maintain monorails for underslung hoists in accordance with provisions stipulated in ANSI B30.11-1988, Monorails and Underhung Cranes, Paragraphs 11-1.3.1(g), 11-2.1.3 and 11-2.3.3(a). Essential elements include the following: 1) Assure that mechanical stops on monorails resist impact forces of a fully loaded carrier or hoist traveling at normal walking spped or at least 50 percent of the rated full load speed if the carrier or hoist is power operated. 2) Perform periodic inspections and make necessary adjustments and/or repairs before resuming operation of the hoists. 3) Assure that proper, acceptable rigging devices are used and installed in the recommended manner.
