29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Throughout facility: The employer did not ensure that employees utilized the lockout tagout procedures when cleaning, servicing or maintaining equipment, including, but not limited to, the doughnut cutters, Belshaw Thermomatic proofer boxes model TM600#23932 serial 1404045 and model TM600 # 22584 serial 14030178, Belshaw Century Gas fryers model C10-24GLH#23929 serial 14030180, Hobart Mixers and Berto Yeast Mixers model 160-AR machine number 171330 and model 160-AR machine number 171329, on or about 10/5/15. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
