29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the forks and that the forklift was not moved horizontally while the platform was occupied. On or about 25 September 2015, four employees were working from a job-site made wooden trash box on the forks of a rough terrain, telescoping boom forklift up to approximately 19 feet high without the employer ensuring that the platform was attached to the forks, exposing the employees to a fall hazard.
