29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Cardington Yutaka Technologies, Inc. on or about September 09, 2015, at the address of 575 West Main St. Cardington, OH: The employer failed to ensure that employees who are responsible/tasked with service work at or near the point of operation on the Double End Forming Machine (#320052) were adequately evaluated for the skills required for the appropriate application and usage of machine specific energy control procedures. Employees performing service work in an energized condition were exposed to amputation hazards.
