29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: On or about September 14, 2015, employees were exposed to trip hazards in that: a. There is a hole in the floor with a partial loose grate covering it next to the wall at the grinder control box b. There is a hole in the floor at the drain in the aisle between lines 1 and 2. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
