Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Employees who performed servicing and maintenance on Great Lakes TS-37 and ARPAC TS-33 packaging machines were not required to: (2) turn off and shut down the machines, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device, (5) prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment had been accomplished. (a) Employees required to change film rolls on Great Lakes TS-37 packaging machines were exposed to rotating shafts creating entanglement hazards from unexpected start-up and energization. (b) Employees required to clean the sealer bar on ARPAC TS-33 and Great Lakes TS-37 packaging machines were exposed to the pinch point and thermal hazards created by the machines unexpected start-up and energization.
