Text For Citation: 01 Item/Group: 002 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by pipe coupling device: a.IN THE PLANT, BUILDING TR-1, J-BAY: On or about November 28, 1993, employees exposed to being struck by a Victaulic grooved pipe coupling device, model type Style 77. Coupling device was attached to an end cap in order to perform a hydrostatic pressure test on a 17FA Condenser. Operating manual and specific training were not provided to employees prior to installing coupling device as required by the manufacturer.
