29 CFR 1910.178(l)(4)(ii)(B): Refresher training and evaluation was not provided to the operator when the operator was involved in an accident or near-miss incident. a) On or about 08/25/15, in the facility at 44-28 55th Ave, Maspeth, NY An employee was standing on an elevated platform (pallet) attached to the forks of a powered industrial truck retrieving inventory from the 2nd level of a 4 level metal storage rack and fell approximately 10 ft. to the concrete floor. ABATEMENT DOCUMENTATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
