29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done: On or about July 28, 2015, in Area 2 behind the Hang Line where chicken was being cut by a 9" blade on the Cantrell KFC cut-up saw machine, Model CS-100, the employer allowed an employee to operate the saw where the blade becomes unguarded as the chicken is pushed into the blade for cutting specialty parts. The employer could not produce any copies of its "Job Safety Assessment Form" (JSA).
