29 CFR 1926.651(e): Employee was not prohibited to be underneath loads handled by lifting or digging equipment: On or about August 12, 2015, at a job-site located at 3450 Adaline Dr, Stow, Ohio 44224; the employer did not ensure employees were protected from falling loads by allowing them to work under the Caterpillar, wheeled front end loader, Model IT38, Serial Number 6FN00539 while sand was being dropped. Employees spread the sand to raise the floor level of the trench as part of the company's standard operating procedure.
