29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: During the week of November 2, 2015, and prior, the employer failed to ensure employees directed to perform preventative maintenance were protected from various fall heights, including but not limited to, the following cranes: a. An approximate 40 foot fall while gaining access to crane 315. Employees walk from crane 310/318 across the crane runway and enter the crane without utilizing a personal fall arrest system. b. An approximate 40 foot fall by ensuring employees do not have to climb over equipment on the east side which takes them above the guardrails of the trolley on crane 304. c. An approximate 9 foot fall by ensuring employees do not have to climb over equipment on the southeast entry area which takes them above the guardrails on of the trolley of crane 307. d. An approximate 40 foot fall by ensuring employees do not have to climb over equipment on the north side rail which takes them above the guardrails on the trolley of crane 305. e. An approximate 40 foot fall by ensuring employees do not have to climb over equipment on the south rail side which takes them above the guardrails on the trolley of crane 307. f. An approximate 9 foot 6 inch fall by ensuring employees do not have walk in areas that take them above the guardrails on the southwest corner on the trolley of crane 314. g. An approximate 40 foot fall by ensuring employees do not have to climb over equipment on the west side rail area on the trolley of crane 305.
