29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces were not protected from falling through holes more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) Centimark Corporation, 319 Industrial Drive, Franklin, Ohio: On or about October 12, 2011, at the worksite located at 1820 McClain Road, Lima, Ohio, the employer did not ensure that employees performing work on the roof and the membrane roofing were protected from falling through a hole in the roof created when the roof-mounted sliding door was open. An employee was exposed to a 65 feet fall through a hole in the roof. No warning lines or other safety devices were used to alert the employee to the hazard. The employee was not wearing fall arrest or restraint equipment.
