Text For Citation: 01 Item/Group: 002 Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential injuries from elevator collapse: (a)Freight elevator was not installed in accordance with American National Standard Safety Code for Elevators (ANSI A 17.11971) NOTE: See appropriate rule noted below: 1.Chain was used to for suspension of elevator car in lieu of steel wire rope. See Rule 212.1. 2.A single chain was used to hoist car. The minimum number of wire ropes shall not be less than two (NOTE: Minimum diameter shall be onehalf inch). See Rule 212.4. 3.Safety devices to prevent free fall were not installed. Safeties shall consist of a device which applies pressure to steel guide rails and activated based upon a set governor speed. See Rule 205.1 and 205.5. 4.Elevator shaft was not equipped with steel guide rails. See Rule 203.2. 5.Elevator car was not enclosed. Car sides and top was enclosed with chain link fencing material. See Rule 204.1a. 6.Elevator car was not equipped with car doors. And landing gate at the first floor was not equipped with a functioning electrical contact. See Rule 204.4. 7.Car capacity was not posted. See Rule 301.10 and 207. 8.Switches are not fail safe. Single switches are used to control interlocked door/gate. In case of failure of door or gate switch the car could start or run. See Rule 306.9c. NOTE: This is not a conclusive list of the deficiencies for this elevator. Among other methods, one feasible and acceptable abatement method to correct this hazard is:HAZARD IS: 1.Replace elevator in accordance with current ANSI A17.1 series, or 2.Remove elevator from service.
