29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment: a. At the site, the employer did provide training to the employees for the use of personal protective equipment while performing tree trimming tasks. An employee was working from a vehicle mounted aerial lift and was not using fall protection. On or about September 6, 2011 an employee fell from a vehicle mounted aerial lift while trimming a limb from a crabapple tree at a residential yard and sustained fatal injuries to the head. The employee was not utilizing a body belt and harness while performing tree trimming tasks in the vehicle mounted aerial lift.
