29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels was not protected by guardrails systems, safety net system, personal fall arrest system (PFAS), or other approved alternative fall protection systems: (a) Peak to Peak Roofing & Exteriors, LLC @ 1541 South Patton Court, Denver, CO 80201. On or before September 22, 2011, the employer did not ensure that fall protection systems were utilized by roofing subcontractor employees while working from a 4-in-12 roof on two residential homes. This condition exposed the employees to a fall hazard of approximately 11 feet eave to ground.
