29 CFR 1910.178(m)(1): Trucks were driven up to employees standing in front of benches or other fixed objects: (a) Receiving dock -- On or prior to 5/14/15, lift trucks were driven up to employees standing in front of a downstroke baler which was positioned between dock doors 2 and 3. On 5/14/15, an employee operating a down stroke baler was severely injured when he was struck by a forklift as the forklift operator attempted to pass by.
