29 CFR 1910.212(a)(3)(ii): The point of operation of machines, whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. Production Department - On or about April 18, 2015, the employer did not have the point of operations adequately and completely guarded, in that the Formax, Model F6, Serial number 039, was not adequately guarded, thereby exposing employees to amputations. b. Production Department - On or about April 18, 2015, the employer did not have the point of operations adequately and completely guarded, in that the Formax, Model F6, Serial number 245, was not adequately guarded, thereby exposing employees to amputations. c. Production Department - On or about April 18, 2015, the employer did not have the point of operations adequately and completely guarded, in that the Formax, Model F6, Serial number D6FN54, was not adequately guarded, thereby exposing employees to amputations. d. Production Department - On or about April 18, 2015, the employer did not have the point of operations adequately and completely guarded, in that the Formax, Model F6, Serial number D7GT05, was not adequately guarded, thereby exposing employees to amputations.
