29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. On or about April 23, 2015, the employer failed to ensure employees cleaning chips from the AmeraSeiki CNC, model number VB-2, serial number 2010028, placed a locking device on the equipment to ensure no accidental startup. b. On or about April 23, 2015, the employer failed to ensure employees cleaning chips from the Phoenix CTC Copying Milling Machine, model number VTC-30-10, serial number 131056, placed a locking device on the equipment to ensure no accidental startup.
