29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and were not done in the following sequence: Employees who performed servicing and maintenance on the following equipment were not required to: (1) Turn off and shut down the machines, (2) Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (3) Affix lockout or tagout devices to each energy isolating device, (4) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished. On the following servicing activities lockout was not completed: A. On 6/29/15, an employee clearing jams on the full revolution mechanical power press EA5324 located in the center fire primer line. B. On 5/29/15, an employee clearing a jam and was adjusting the trigger on the die of the full revolution mechanical power press #20, located in the shot tower/swager area. C. Employees corrected tube arm misfeeds on the part revolution V&O mechanical power press #11 did not use lockout during the week of 4/27/2015. D. During the week of 6/22/2015, employees cleared misfeeds on the part revolution mechanical power press #31 without locking out.
