29 CFR 1926.603(c)(5): When it was necessary to cut off the tops of driven piles, pile driving operations were not suspended except where the cutting operations were located at least twice the length of the longest pile from the driver: a) 100 Green St, Marcus Hook, PA - Tank 40-TK-102 The employer permitted the employees to work in close proximity to the pile driving operations while the cutting off of driven piles took place. An employee counting pile blows was fatally injured when he was struck by a falling pile. The pile driving operations were ongoing within approximately 20 feet from the cutting operations. The pile which was cut was approximately 24 feet and 3.25 inches long, on or about March 30, 2015.
