29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tag out device to the group lockout device before working on the machine or equipment: On or about 16 March, 2015 at the cooling towers; employees were exposed to struck-by, caught-in, and amputation hazards where one lock was used for two employees to control the hazardous energy while performing maintenance on equipment including but not limited to the cooling towers.
