29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) On or about 3/10/15 - at Units 3 & 4 Cooling Tower Chlorinator, an employee responding to a chlorine leak did not use gloves (butyl rubber, neoprene, nitrile, or PVC) and PVC rain suit to respond to a minor leak as identified in Miller Laboratory Procedures MILL-00-06--1.14 R1, Chlorine Leak Response.
