29 CFR 1926.1430(g): The employer did not provide refresher training in relevant topics for each employee when, based on the conduct of the employee or an evaluation of the employee's knowledge, there is an indication that retraining is necessary: On or about March 11, 2015 and at times prior thereto, a crane operator after his daily crane inspection used damaged slings to lift a steel beam grid. The employer did not provide training to enable employees to recognize the hazards and procedures to follow in order to minimize these hazards. Employees were exposed to crush-by, struck-by and caught-between hazards.
