29 CFR 1910.147(f)(3): When servicing and/or maintenance was performed by a crew, craft, department or other group, the employer did not ensure that employee(s) utilized a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device: Employees engaged in maintenance activities at Compressor C-4 in the Ammonia Room were exposed to electrical shock and chemical hazards in that personal locks for each worker were not affixed to the hasp of the compressor being serviced by a group of employees.
