29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard: a. On or about March 3, 2015, on the concrete drive between the processing building and the holding warehouse that is used by forklift traffic, the metal grating over the drainage channel was damaged and did not fully cover the recessed drainage channel, exposing employees to a trip hazard and forklift rollover hazard.
