29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o). a) On or about March 26, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, while operating the Cantilever Band Saw (end/left) the operator was exposed to an unguarded rotating pulley and belt used to power the saw blade. b) On or about March 24, 2015, in the Dyson Forge Threading (DFT) Division located at 53 Freedom Rd., Painesville, while operating the MA-4 NU Auto Nut Tapper(number one) the operator was exposed to an unguarded rotating pulley and belt. c) On or about March 24, 2015, in the Dyson Forge Threading (DFT) Division located at 53 Freedom Rd., Painesville, while retrieving parts form the storage cabinets behind the MA-4 NU Auto Nut Tapper(number one) the machine setup employee was exposed to an unguarded rotating pulley and belt.
