29 CFR 1910.119(d)(3)(ii) The employer did not document that equipment complies with recognized and generally accepted good engineering practices: a)Mahantango Site, main ammonia engine room, the door leading from the engine room interior to the operator control room was not self closing and the employer did not document that the equipment complied with recognized and generally accepted good engineering practices, on or about June 18, 2015. b)Mahantango Site, main ammonia engine room, a household window air conditioner was mounted in the wall between the engine room and the operator control room and directed engine room air directly into the control room and the employer did not document that the equipment complied with recognized and generally accepted good engineering practices, on or about June 18, 2015. c) Mahantango Site, O Cellar, two tube and shell heat exchangers were located in an occupied process and production area and an ammonia detection system was not installed to provide employees with an early warning in the event of a leak, on or about June 18, 2015.
