Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The empployer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Medical Arts Building, 185 North Wabash Avenue, Chicago, illinois - Employees were exposed to hazards of being struck by malfunctioning elevator cars, controls and safety devices: (1) Periodic maintenance and service inspections were not implemented on the freight elevator, number 6 to ensure that all safety devices including interlocks for car doors are in operable condition. Among other methods, one feasible and acceptable method to cor- rect this hazard is to establish a periodic maintenance and in- spections tests to ensure that all safety devices to the eleva- tor are in an operable condition as stipulated by ANSI/ASME A17.1-1987, American National Standard, Safety Code for Eleva- tors, Escalators, and Moving Walks.
