29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a.) Shiloh Industries, Inc. dba Wellington Stamping located at 350 Maple St. Wellington, OH 44090; The employer did not assure the 01C Danly Mechanical OBI Power Press was deenergized before clearing jams. On January 28, 2015 an employee sustained a crushed hand and three finger amputation when he reached in to clear a jam on the 01C Danly Mechanical OBI Power Press and another employee cycled the press.
