Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) On or about January 21, 2015, in the north side of building #2 of the facility, employees were exposed to serious struck-by and or crushing injuries, in that the employer had not developed, implemented and utilized Specific Energy Control Procedures for the Overhead Bridge Crane that was in operation. Maintenance activities were being performed on an overhead light fixture that was in the travel path of the crane which had not been locked and tagged out. As a result, one employee was fatally injured when his elevated man lift was struck by a moving crane. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
