29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition: a) Job site, 650 Braddock Avenue, Pittsburgh, PA: On or about January 22, 2015, during production activities, the employer did not ensure that the stairs leading up to work platforms were kept clean in that the stairs were covered with fine dust that made them slippery. b) Job site, 650 Braddock Avenue, Pittsburgh, PA: On or about January 22, 2015, during production activities, the employer did not ensure that the floors were properly maintained in that floor area had many tripping hazards caused by materials/structures in the floor and/or damaged concrete areas.
