OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush-by hazards: (a) On or about January 15, 2015, and at times prior, employees worked and operated equipment in the post crop room, the phase II rooms, and the dry storage room #2, where the structural stability of the concrete panel walls and concrete roof had not been verified after damage to the concrete panels, exposing employees to crush-by hazards. (b) On or about January 15, 2015, and at times prior, employees worked and operated equipment in the wharf and Ricking areas, where the structural stability of the supporting and connecting steel beams had not been verified after damage to the support structure, exposing employees to crush-by hazards. Colorado Mushroom Farm, LLC. formerly known as Rakhra Mushroom Farm Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 588999, citation number 1, item number 2 and was affirmed as a final order on January 4, 2013 ,with respect to a workplace located at 10719 Road 5 South, Alamosa, CO 81101.
