29 CFR 1926.454(a)(3): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training did not include the proper use of the scaffold, and the proper handling of materials on the scaffold: (a)Denali Roofing LLC at 184 W. 64th Ave., Loveland, CO 80538: On and before January 14th, 2015, the creating, exposing, and correcting employer did not adequately train an employee when elevating on an articulating aerial lift, JLG Model 450AJ, SN 0300177390, to set metal panels on the exterior of this building. The condition exposed this employee to fall hazards of up to 16 feet.
