Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to THE HAZARD OF FALLING INTO THE ELEVATOR SHAFT AND/OR BEING CRUSHED BY THE ELEVATOR CAB. a)COLD MILL DEPT., 88" MILL INTERMEDIATE OIL MILL CELLAR, ON OR ABOUT 08-08-96, EMPLOYEES OPERATING MATERIAL ELEVATOR, AND/OR WORKING IN THE VICINITY OF A ELEVATOR WHICH HAD AN INOPERATIVE MECHANICAL INTERLOCK ALLOWING THE SECOND FLOOR LEVEL PLATFORM DOOR TO OPEN WHEN THE ELEVATOR CAB WAS NOT AT THAT LEVEL. ABATEMENT NOTES: AMONG OTHER METHODS, ONE FEASIBLE METHOD AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD IS TO: 1.REPLACE AND/OR REPAIR MECHANICAL INTERLOCK AT THE SECOND FLOOR LEVEL. 2.THE INTERLOCK DEVICE SHALL HOLD THE DOOR IN THE LOCKED POSITION BY MEANS OF GRAVITY OR BY A RESTRAINED COMPRESSION SPRING, OR BY BOTH, OR BY MEANS OF POSITIVE LINKAGE.
