29 CFR 1926.451(e)(6)(iv): Integral prefabricated scaffold access frame rungs were not uniformly spaced within each frame section: Inside the structure under construction: The integrated ladder rungs on the JLG scissor lift, model number 2646E2, were not uniformly spaced as the second rung from the bottom was damaged and bent upward. Employee's using the ladder to access the lift were exposed to slip and fall hazards.
