29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used were not guarded by a floor opening cover or protected on all sides by removable standard railings: The employer does not ensure standard railings are used when pits are left uncovered, exposing employees to the hazard of falling into the open pits. This violation was observed on or about December 1, 2014 in a narrow area between the metalizing area and sand blasting bay, employees were exposed to a fall of approximately 47 inches into an uncovered /unguarded pit. Pursuant to 29 CFR 1903.19, within (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure employees are protected from fall hazards.
