29 CFR 1910.26(c)(1): Good safe practices in the use of ladder equipment were not employed by the users to get maximum safety: a) On or about 12/04/14 at the Thionyl Chloride bulk storage tank; employer failed to protect employees from fall hazards when they allowed the use a twelve foot single ladder, placed against the side of the storage tank with the base of ladder located outside the dike wall ( creating an approximate pitch of 50 degrees ), to open and close the valves on the top of the tank. ABATEMENT DOCUMENTATION REQUIRED
