29 CFR 1910.268(o)(1)(i): When covers of manholes or vaults are removed, the opening was not promptly guarded by a railing, temporary cover, or other suitable temporary barrier which is appropriate to prevent an accidental fall through the opening and to protect employees working in the manhole from foreign objects entering the manhole: a) Intersection of Main St. and Boland Way, Springfield, MA: On or about October 21, 2014, employees were working about manhole(s) whose cover had been removed and were not subsequently guarded by any means to prevent employees from falling in the open manhole(s) or prevent foreign objects from entering the manhole(s) and injuring the employee working in those manhole(s).
