OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to electrocution from a 3-phase 4.16kv and a 480 Volt electrical line that was not grounded. On or before October 16, 2014 at the Metal Shredders, Inc. DP&L/Metal Shredders, Inc. 4.16kv dual-access substation, a site supervisor was exposed to an electrocution hazard where after testing the three phase 4.16kv electrical lines inside Cabinet #3 and at the 480 Volt AC industrial substation, did not install temporary grounds to remove the potential of electrical voltage differences in the substation. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
